1. Hotel reservation conditions

• All delegates are responsible for their travel and accommodation arrangements. EPOS does not guarantee hotel rooms availability for all participants. However, rooms have been pre-blocked by EPOS at preferential rates at Hilton, Mercure and Campanile hotels. These special rates are only available when the booking is made through MAZURKAS TRAVEL before 18th February 2008.



• To make a hotel room reservation, use the On-line Reservation System. If you pay both by bank transfer or by credit card you must add to your registration fee a mandatory hotel payment covering the first night at Hilton and Mercure hotels and covering the full stay while the reservation is made at Campanile hotel. Additional booking fee of 10 EUR is added to the first night payment whatever the hotel is.



• Rooms will be allocated on a first-come-first-served basis. Any change of a reservation should be made in writing to MAZURKAS TRAVEL and not directly to the hotel.

IIN CASE OF NO-SHOW AFTER ONE NIGHT, THE HOTEL WILL CANCEL YOUR
RESERVATION AND CHARGE YOU ACCORDINGLY.

2. Hotel rates (in Euros - €)

Hilton Hotel ***** - The Congress Venue
	This luxurious hotel, situated in a business Warsaw’s centre, has more than 90 m. height. There are 314 rooms in the building, which are equipped with air-conditioning, Internet, safe and television LCD. Guests are able to use perfectly prepared recreation center: Fitness Club $ SPA with 25 – metres swimming pool. Visitors have a possibility to choose here many restaurants, for example: MEZA – which specialized in international kitchen, PISTACCIO Lobby Bar & Lounge and bar – club AXIS.
Mercure Hotel ***
	The Mercure Hotel is located in the centre of Warsaw, on the one of the city’s main streets, conveniently situated within walking distance from the Old Town and railway station. It is 12 km far from the Okecie International Airport and only 2 km from Central Railway Station. It offers 240 rooms, including 6 rooms for the underprivileged and 7 suites. Every room includes radio, TV SAT, telephone, mini-bar, safety deposit box, coffee and tea pots, hairdryer, air-condition and access to the Internet.
Campanile Hotel **
	Located in the heart of city center about 8 minutes by walk from Central Railway Station, and about 7 km from Okecie Airport. Calm, spacious and comfortable rooms. The rooms are equipped with air condition, bathroom with bath and shower, hairdryer, wake up call, coffee, tea, biscuits, cable TV and Canal +, work desk, direct telephone line and computer socket for internet connections.

Rates are given in EURO.

All rates are per room, per night, including breakfast, service and VAT.

The first night rate includes booking fee of 10 EUR.

Early reservation is highly recommended!

Each apply for a room will be treated as an order. If there is still possibility to book a room a ‘Notification of order receipt’ will be sent to the participant and the reservation will be made. Nevertheless a room booking will only be guaranteed if accompanied by a payment, which will be either charged to your credit card or advanced by bank transfer.

In case of Hilton, and Mercure hotels the rest of the payment for the room must be paid by each delegate directly to the Hotel upon departure. The Hotel will deliver the appropriate invoice covering the balance.

In case of Kyriad Prestige and Campanile hotels total payment for the whole reservation shall be effected to MAZURKAS TRAVEL in advance.

Please, kindly note that after 18th of February every change in the hotel booking is subject to 15 EUR administrative fee.

3. Hotel room confirmation

Confirmation of the hotel room reservation will be sent by MAZURKAS TRAVEL to each delegate as soon as the registration and hotel reservation form as well as the payment for first night or full stay accordingly reach the Mazurkas account.

Cancellation of Hotel Reservation

Cancellation of your hotel room must be notified in writing to MAZURKAS TRAVEL. Please note that if you cancel your room.

• before 18th February
  No penalty fee will be charged on credit card and the full refund of your hotel deposit (minus € 15 to cover administration costs) will be reimbursed.
• after 18th February
  a minimum of first night payment (i.e. your hotel deposit) will be charged by Mazurkas.

  Please, note that the account beneficiary is MAZURKAS TRAVEL B.P. Sp. zo.o. All bank transfers send to EPOS (European Orthopaedic Paediatric Society) will not be accepted.
